Has anyone been successful in integrating fall risk screen/assessment and referral into primary care EMR?

In the North East LHIN we are working with a group of Family Health Teams who are keen to integrate a fall risk screen/assessment and referral tool into their electronic medical record (EMR). We are basing the algorithm off the American/British Geriatric Society Clinical Practice Guidelines (The American Geriatrics Society, 2011). Champlain LHIN has been working on a similar tool. The STEADI algorithm is the one we have selected to pilot test. Public Health Units are working to identify the best practices resources to accompany the EMR. What are the best practice resources you use for: Visual acuity; postural hypotension; etc?
