Has anyone developed a Clinical Practice Guideline or similar documentation for falls prevention targeted to the Palliative Care population?

We are in the process of developing a tool based on acute, long-term care and community resources to target falls prevention in the palliative population and we are curious as to where the community at large may be with this? We were wondering if someone has developed something if they would be willing to share too?
