Falls are preventable statistics/reference?

Hi all, I was approached by Toronto Public Health asking if I am aware of a reference to support the statement that 95% of falls are preventable. This is not something I can locate, but thought I would turn to the Loop community as you guys are very knowledgeable. Is anyone aware of where this might stem from?
