AFC/Fall Prevention Calendar

The South East Regional Integrated Fall Prevention & Management Strategy in collaboration and support from members of the Ontario Age-Friendly Communities Outreach Initiative developed a 14 month calendar to promote awareness of fall risk factors, prevention strategies and age-friendly domains. The group has been disseminating 5000 copies across South East Ontario to older adults through a number of organizations providing services across the continuum of care. We are planning evaluation activities in the New Year with older adults and are welcoming health care provider feedback as well. We hope once we have incorporated feedback this is a health promotion tool that can be adapted and disseminated across the province. We welcome your thoughts.
