Looking for a falls screening tool for EMR (Family Health Team)

Stepping Out Safely Perth-Huron is a coalition of community organizations, allied health care professionals, primary care organizations, hospitals, health units, retirement homes and volunteer organizations. Our goal is to reduce falls and fall injuries among older adults in Perth- Huron through education, skills building, community collaboration and policy development. The Health Care Provider Engagement Sub - Committee is looking for a screening tool adaptable to an EMR . The plan would be to pilot it with a couple of family health teams.
