Nursing Assessment

Hi,

I am seeking out a standardized nursing assessment for individuals who have been identified as a high falls risk. Wondering if anyone has an assessment or assessment tool they could share? Also intererested in hearing from any teams that incorporate multiple health care professionals (i.e. OT, PT, RN) into an individual falls prevention assessment and the roles of each health care professional.
