Ontario Fall Prevention Collaborative Screening and Assessment Survey

It is November, Fall Prevention Month, and the Ontario Fall Prevention Collaborative needs your help!

Do you work in Ontario, in one of these health care sectors: 

Public Health, Home and Community Care, Primary Care, Emergency Dept, Paramedic Services , Urgent Care.

 

If so, please assist us in understanding the use of screening and assessment tools for falls risk by practitioners in these areas of work by completing this survey. It should only take about 10 – 15 minutes of your time and will help to inform us as we progress with the work on the priorities set by the members of the Collaborative.(https://onf.org/implementation/prevention/).

We would especially welcome your participation in future focus groups when we look at validating the survey findings and digging deeper into screening and assessment to identify issues with utilization and implementation in these tools in practice, so please click yes to that request in the survey.
