Multifactorial Risk Assessment checklist

I am looking for resources on checklists our community paramedics can use when doing a multifactorial risk assessment for falls - something newer members can use to help guide them when doing the assessments to avoid missing critical helpful assessments. 

- physical exam elements

- home safety items to assess

- footwear and feet exam elements

Rather NOT re-invent the wheel
