Environmental Hazard Assessments for Fall Prevention

I am compiling a list of environmental fall risk assessment tools/approaches currently being used in fall prevention programs that are aimed at seniors living independently in the community. 

I would be most grateful for information about environmental falls risk assessment tools/approaches currently in use anywhere in Canada. I am especially interested in risk assessment tools that include the bathroom and/or stairs/steps (either indoor or outdoor).
