Risk mitigation with discharge planning

I am an educator working with new grads in acute and rehab settings and they are often expressing difficulty with the grey zone of "discharging at risk" and struggle to know when to keep clients longer in acute/rehab vs discharging at risk with community/outpatient follow up. Does anyone know of existing frameworks that can help with this decision-making?
