Does anyone have a post-fall assessment tool or information on post-fall information to share with home care nurses?

I'm looking for a teaching tool to share with First Nation and Inuit home care staff to use themselves in assessing client need, or for client teaching to reduce further falls or complications of previous fall. thank you
