I’m looking for any information on effective ways to collaborate/share info with Physician offices, FHTs, NPs etc.

Our health unit (in Oxford) is hoping to form a central group (similar to a Community of Practice) to share health info in a more efficient way. Currently our different departments – VPD or CD or Sexual Health etc - all contact separately. This makes it more difficult for Population Health to make any connections as the offices get a bit inundated with PH. We’re hoping we can form a group to meet a few times a year either through meetings or online and use that forum to offer updates/info on various topics of their interest…and hoping fall prevention will be one of those interests!

If there is another health unit/hospital/organization that has done something similar, I would love to hear from you! (Especially if your area covers rural areas.)
