Does anyone have a process in primary care that engages or includes the PT and OT?

After the last webinar on fall prevention in Primary Care, I think it is really important to have OT and PT involved. Whether it's in the primary care centre or through a referral. OT and PT could be doing home assessments and prescribing exercise for patients. Does anyone have a process in primary care that engages or includes the PT and OT?
